How Do I Use It?
Before you start the request please make sure you have all of the corresponding information pertinent to the Employee Termination request. (Previously available on the Employee Term Form) Including any applicable documentation to attach to the request.
Once you're prepared to submit your request, click the "Complete Form" button on the right hand of this screen to open the form. Complete all required fields and click the "Request" button at the bottom of the screen.
